1. Go to passport.com.ph website to fill up the individual application form and secure a schedule for your appointment at the DFA. Make sure you have a working email because DFA will send your reference number and application form by email.
2. Secure all documents required for your passport, One of the most important of these documents is your birth certificate. You need to get a copy of your birth certificate in security paper (SECPA) from the the National Statistics Office (NSO).You also need a valid ID. Your ID is preferably a digitized government-ID such as those issued by the SSS and PRC.For students, a valid school ID will do. Supporting documents may also be required and these documents should have your full name, date of place and birth and citizenship. Your supporting document may be any one, two or three of the following:

Marriage Contract Land Title Driver’s License Government Service Record School Form 137 or Transcript of Records with dry seal Other documents that show full name and birth details of applicant and/or citizenship Voter’s Registration Record from COMELEC Intramuros Baptismal Certificate with dry seal Seaman’s Book Income Tax Return (Old) NBI Clearance

The DFA Passport Appointment System is initially available for

1. those whose passports have already expired;

Passport applicants can apply for an appointment by sending a request to appointment@dfa.go.ph with the following details:

• Full name (first name, middle name and surname);
• Date of birth;
• Place of birth;
• Latest Passport Number and Date of Issuance

Passport applicants will then be sent a reference number and a confirmation which will include details of designated date and time for the filing of the passport application at the DFA-Passport Office.
